The Wyoming Medical Power of Attorney Form is a legal instrument intended to facilitate the grant of powers to take health care decisions on behalf of the principal signing this tool even if he is incapacitate due to whatsoever and/or unforeseen reasons. 2014 Wyoming Statutes, Title 35, Chapter 22, and Article 4 – Wyoming Health Care Decisions Act govern the issuance and execution of the said power of attorney. The “attorney-in-fact“, appointed through the execution of the power of attorney is in a position to procure medical care for the signing principal by completing documentation, releasing authorization/s, and perform any act, right, power, obligation, or duty as and when needed.
Notarization of the signature of the principal signing this Medical Power of Attorney before the Notary Public of Wyoming State is mandatory for its execution and legal standing.
Wyoming Medical Power of Attorney Facts
- You as the principal signing the Medical Power of Attorney must specify the date of execution of the same as per the provision in the document.
- Issuance and execution of this legal instrument supersedes/revokes all previously issued power of attorney/s by you in any and every form.
- Attorney in fact undertakes to provide required medical care to the principal in his/her best interest and is therefore granted the power to contract with the hospital, physician, or any other health facility as and when required.
- This Medical Power of Attorney does not authorize financial transactions on behalf of the signing principal.
How to Fill the Medical Power of Attorney correctly
Step 1: You as the signing principal must furnish the information truthfully. Specify your name and address along with the legal name of the attorney in fact.
Step 2: You must mention the date of execution of this Medical Power of Attorney using the provision for the same.
Step 3: Insert date and sign in the space reserved for your signature. You must sign before the Notary Public.
Step 4: Continue with the notarization by the Notary Public of Wyoming State.
Terminating this Wyoming Medical Power of Attorney
This legal instrument construes and grants indefinite powers to the attorney in fact unless and until it is terminated in writing by you as singing principal. You may cancel the power of attorney fully or a part of it. The revocation of the Medical Power of attorney is governed by WY Stat § 35-22-404 (2014) and you may select any of the five causes that instigate revocation of the power of attorney as specified in WY Stat § 35-22-404 (2014).
